 Post subject: PA Long Form Birth Certificates?

Before discovering this forum, I ordered all of my ascendants' Pennsylvania birth certificates from VitalChek.com. As instructed by their website's FAQ, I was careful to choose Dual Citizenship as my reason for ordering the documents. According to the FAQ, the agency would send me the appropriate form of the certificate based on the reason selected.

All of these docs have since been apostilled and are awaiting my June 2011 SF appt. Upon further scrutiny, I'm wondering if VitalChek sent me long form certificates... :?

malcolm and katier, I read here that you also had PA docs in your application. Could you (and anyone else who has successfully submitted PA application docs) please let me know if the following seems like a long form doc? (I called the PA Dept. of Health several times only to receive busy signals and hang-ups when I try to get an operator/rep.) Thank you!

Yes - it's a half sheet doc with those fields, signed by the state registrar, official certified copy.
I was able to get apostilles on these. I'll find out on 5/7 (next Friday) if this is the correct document.

The only Penn "certificate" that I had was a No Record letter for my GM's birth certificate, since they couldn't locate it.

I have to say, that long form that Katier posted looks more like a short form from other states, but it seems to include all the necessary information. I guess we'll find out if it's good enough in a couple of weeks.

I kept scanned copies of all of my documents, too, but not in color. :P

katier, thanks for posting your scan - that's exactly what I have. Wishing you the best of luck with your appointment, and fingers crossed we have the right birth certs!

malcolm, that was my impression, too: my long forms looked a little too short... Oh well, at least I'll have plenty of time to fix them if they end up being short. :|

I sure hope it's the right document form. My older son was born in PA - and it's the only official form of birth certificate I received following his birth. (The hospital had a very unofficial form - with it noted that it was NOT official.)

I just did some more googling I discovered:
- It's the only form that is issued by the State of PA.
- If you go to the county - it bounces you to the state DOH website for birth cert requests
- If you google "long form birth cert" you will get zillions of sites obsessing about Obama's birth certificate, various copies of it, etc. (Not being politcal here - just stating a fact... google and see.) This made it more challenging to find useful information.

I'm sure I'll be posting about our appointment sometime 5/7 or 5/8... Rafaella or whoever helps us in LA will be the person who knows.

OMG, the Obama birth certificate search results... I did a ton of googling to no avail before asking here and couldn't believe that no matter how precise my search criteria, the top results were regarding Obama's birth cert, including image searches. Very interesting.

Thanks for the additional info. I feel better, as I have 4 PA birth certs all in the aforementioned format.

I had 2 PA BC's in my application which were in identical format as Katier's. They were accepted in LA without a problem.

There's a way to do Google searches where you exclude certain terms. For example, you ask for pages with "long form birth certificate" but which don't include "Obama." Try this: search for: long form birth certificate -Obama. Or just use the advanced search feature, which does the same thing.

That would be -Obama. Everthing barring that seemed to yield an Obama result. Even omitting Obama didn't help much, but I was impatient because I couldn't immediately find an example of the PA long form... This board, however, has been extremely helpful!
